Performance of a disposable colonoscope for routine examination: Pilot randomized controlled noninferiority trial (with video)

  • This randomized controlled noninferiority pilot study compared the technical performance of disposable versus reusable colonoscopes in routine colonoscopies. Among 116 patients, both groups achieved a 100% colonoscopy completion rate, confirming the noninferiority of disposable colonoscopes. While disposable devices showed slightly lower performance in imaging and maneuverability, clinical outcomes and safety measures were comparable, suggesting that with continued refinement, disposable colonoscopes could become a practical alternative in specific clinical contexts.

The Genetic Architecture of Congenital Diarrhea and Enteropathy

  • This study analyzed the exomes or genomes of 129 infants with suspected monogenic congenital diarrheal disorders (CODEs), identifying causal variants in 48% of cases, including a novel founder variant in NEUROG3. Functional studies using cell and zebrafish models led to the discovery and characterization of three novel CODE-associated genes: GRWD1, MYO1A, and MON1A. These findings expand the genetic landscape of CODEs and support the development of precision therapies guided by molecular diagnoses.

Nonoperative Management of Mismatch Repair–Deficient Tumors

  • This phase 2 study investigated neoadjuvant PD-1 blockade with dostarlimab in patients with early-stage mismatch repair–deficient (dMMR) solid tumors eligible for curative surgery. A clinical complete response was achieved in 84 of 103 patients who completed treatment, allowing 82 to avoid surgery, with a 2-year recurrence-free survival rate of 92%. These results support the potential of immunotherapy to enable organ preservation across various early-stage dMMR tumor types without compromising curative outcomes.
